Moving? How to Start/Stop Service

Please complete and return an Application or Termination Form. (We require a signature so applications and terminations for service are not accepted over the phone.)

Form Instructions:

Complete two forms if you are moving and have a current account with us and will need new City of Platteville service for your new address. (We need one form for each service address.)

Date to read meter: This would be the first date of your lease if moving in. Or if moving out, the last day of your lease. Please note that this date can be in the future. If you are late turning in your form, we can only go back to as far as the first of the current month. We are able to read meters on non-business days (for example, if your lease ends on a Saturday or a Sunday).

Section 1 - new responsible customer: If you are moving in, this is your information. If you are moving out and are a tenant, this will revert to your landlord unless you know the new tenant’s information.

Section 2 - previous responsible customer: If you are moving in, you can leave this section blank. If you are moving in and turning in the form late, and should actually be paying the previous bill, you can put your address for the final bill to be sent to you. If you are moving out, complete section 2. You can leave section 1 blank if you do not know the new tenants as this will revert to your landlord.

HOME OWNERS: If you are a home owner, we will need to know the new owners’ information in order to terminate the account in your name. If you do not know, please indicate the realtor's name so we may contact them.

Signatures: Owner, Agent, Landlord or tenant may sign. Notice will be sent to the other parties involved.

Note: If you are terminating your account at the end of a month or beginning of the next month, you will get two bills due to our billing software. Only pay the billing that reads “final bill”, this bill has both your regular and final bill amounts added together. Remember final bills are “due upon receipt” to avoid a 3% penalty on your previous billing amount. Your final bill is often for 1.5 months of service as we read from the 15th to the 15th. So if you move out May 31st, your final bill will be for water used from April 15th to May 15th plus May 15th to May 31st.

E-bills on Payment Service Network will not reflect a “final bill” amount until the next month. You may still pay the final bill balance online even if the amount is different from the balance shown on your E-bill.
